To ensure a smooth check-in process, please bring the following items to your first appointment:
✔ A current U.S. government-issued photo ID (driver’s license, passport, etc.)
✔ A complete list of all prescribed and over-the-counter medications you are currently taking
✔ Copies of any bloodwork completed within the past 12 months (if available)

Once your initial visit date and time are scheduled, you will receive a link to:

✔ Watch the orientation video, which provides expert insights on:
- How to maximize the effectiveness of your weight loss medication by incorporating small changes into your daily routine
- What factors can make the medications less effective and how to avoid them
- How to begin taking your prescribed weight loss medications safely
- Common side effects and expert-recommended remedies
The orientation video costs $25, but this fee will be applied as a discount at check-out toward your total visit cost. If you have a new patient promotional offer, it will be applied in addition to the orientation discount for even more savings.
