In-Person Paperwork

Step One

Patients are required to update their paperwork and consent forms yearly. Please enter the information in the required fields. PIN numbers expire after 28 days! If your PIN is no longer active, you will need to call our office at (859) 371-4555

Step 2

On the patient portal page, locate the Navigation drop-down menu and click Paperwork. If you are updating your online forms, past information will appear. Make sure that all information is accurate and up-to-date.

Paperwork Completion!

Do not leave the portal until you see the "Paperwork Complete" message to ensure that you have completed all the online forms correctly. Viewing the virtual lecture is required for patients who are new to the practice or have not received treatment here in over 3 years. The orientation fee of $25 must be paid upfront but will be deducted from the cost of your visit at checkout.

On the day of your visit, please bring in the following:

A current US government photo ID

Copies of any labwork that you have had in the past 12 months

A list of all medications including supplements, OTC and prescription

FSA/HSA - You may request an itemized receipt at checkout. Please note that this can be mailed or emailed to you and requests are typically fulfilled within approximately two business days.
